Documents submitted directly to the US Department of State – Office of Authentications requires you to complete Form DS-4194.  This document is available to download from the US Department of State’s website.  Please note that if you decide to mail in your documents to the US Department of State, you will need to contact their office for answers to your questions.

The Department of State, Authentications Office is responsible for signing and issuing certificates under the Seal of the U.S. Department of State (22 CFR, Part 131) providing authentication services to U.S. citizens and foreign nationals on documents that will be used overseas. This office receives a variety of documents from commercial organizations, private citizens, and officials of the Federal and State governments. Documents include but not limited to: company bylaws, powers of attorney, trademarks, diplomas, transcripts, distributorship agreements, articles of incorporation, good standing certificates, home studies, letters of reference etc.

If you are in a rush and would like to have your documents completed in four business days, we recommend that you mail in your documents to our Washington, D.C. office for processing.  Documents received at our office by 10:30am can be submitted the following business morning to the U.S. Department of State for processing.  Once complete, your documents will be dropped off at the FedEx shipping center for delivery in 1-2 business days.

In addition to our to apostille and document certification service, we also provide Embassy and Consulate legalization.  Documents destined for counties who are not members of the Hague Apostille Convention will receive a certificate form the U.S. Department of State instead of an apostille.
